Drug Testing

Bunkie General Hospital is committed to providing a safe, efficient, and productive work environment for all employees. To help ensure a safe and healthful working environment, each applicant to whom an offer of employment has been made will be required as a condition of employment to undergo a substance test. Applicants will be asked to read the policy and sign a Pre-Employment Offer and Employee Consent to Drug Screening.

Full-time or Benefit Employee – 30 hours or more a week (26 pay periods annually)
90 day Probationary Period (effective 2/2015)

PTO (PAID TIME OFF)

Full-time Employees – Accrual begins immediately upon hire however, not
eligible for use until after 90 days of employment (Probationary Period)
Accrual Rate as follows:
0 to .99 years – 4.31 hours per pay period
1 to 4.99 years – 5.85 hours per pay period
5 years plus – 7.38 hours per pay period
Includes paid hours for Vacation, Holidays and Employee’s birthday

SICK TIME

Accrues at the rate of 3.69 hours per pay period with a max of 336 hours
for Full-Time Employees however, not eligible for use until after 90 days
of employment (Probationary Period)
Sick time can be used the second day of illness.

Employer pays for 1X Employee’s Annual Salary up to $50,000

RETIREMENT PLAN

401A & 457 Deferred Compensation Plan (Fiduciary and Combined Plan 
Administration through Nationwide – effective 7/2015)
457 – If selected, Employee must contribute a minimum of 3% of Annual Salary
401A – Employer will match 3% (3 year vesting)
Employee not eligible to contribute until after 90 day Probationary Period.
