Chronic Care Management (CCM) is a service that helps you manage your health if you have two or more chronic conditions expected to last for more than a year. CCM focuses on creating a team-based, patient-centered approach to healthcare.
What is a Chronic Condition?
Examples of chronic conditions include, but are not limited to:
- Alzheimer’s disease and related dementia
- Arthritis (osteoarthritis and rheumatoid)
- Asthma
- Atrial Fibrillation
- Autism Spectrum Disorders
- Cancer
- Cardiovascular Disease
- COPD
- Depression
- Diabetes
- Hypertension
- Infectious Diseases such as HIV/AIDS
- Substance Use Disorders
There are many other conditions that can qualify you for CCM. Based on your condition(s), your doctor or medical staff should know whether or not you qualify for CCM services.

How Can I Get CCM?
